Purify my thoughts, desires, and intentions that I may only desire what is pleasing to you and in accord with your will.”

REQUIREMENTS TO BE SUBMITTED
  1. Download the UPCAT Forms 1 & 2by visiting http://upcat.up.edu.ph/. Correctly and completely accomplished UPCAT Forms 1.  Submit Form 2 to your Guidance Counselor.
  2. Four 2”x2” identical recent photographs taken within the last 6 months.
  3. UPCAT Application Fee or documents for Exemption
a. The non-refundable UPCAT Application Fee is:
(1) P450 for Filipinos with annual gross family income of more than P100,00.
b. Exempted from payment of the application fee are those with annual gross family income of P100,000 or less (supported by 2012 ITRs of earning members of the family or BIR Certification of Exemption);
4.  The application fee may be paid through any of the following (payable to UPCAT):
  1. a.     Any Land Bank (LBP) branch, Account No. 1462-2220-13 (only for peso payments); or
  2. b.   Any Philippine National Bank (PNB) branch, Account No.393496000021 (for peso payments) or Account No. 393496000039(for US dollar payments); or
  3. c.    Manager’s/Cashier’s Check/s (MC/CC) preferably from any of the following:

Deadline of Submission of Requirements (Form 1, 4pcs 2×2 pictures,  If paying thru LBP or PNB, accomplish 3 deposit slips and have them validated, then attach 1 validated slip to your application. If paying with MC/CC, attach the MC or CC (payable to UPCAT) and self addressed stamped envelope onJune 19, 2013 to the Guidance Office.
