Application Procedure for Grade 7 (Day School)
1. Get the Testing & Processing Fee Payment Slip from the Finance Resource Department (FRD) or Guidance Services Office (GSO).
2. Present the accomplished Testing & Processing Fee Payment Slip and pay the non-refundable (Php 150.00) testing and processing fee at the FRD.
3. Present the official receipt to the Guidance Services Office to secure an application form.
4. Fill up the application form and attach the following requirements.
REQUIREMENTS:
- Three (3) pieces 2” x 2” recent identical ID pictures
- One (1) photocopy of Birth Certificate (NSO-SECPA). Applicant must be below 14 years old upon enrolment. (Please bring original copy of Birth Certificate for reference)
- Photocopy of the latest Grade 6 Report Card (Form 138)
- Photocopy of the Parents’ 2011 Income Tax Return (ITR)
- Photocopy of the latest Electric Bills
5. Submit application form and requirements (item #4) to the Guidance Services Office before the scheduled testing date. (Only applications with correct and complete information and requirements will be processed.)
6. Secure your testing permit from the Guidance Services Office upon submission of application form & requirements to the same office.
7. Take the test on your scheduled date. Testing permit must be presented to the assigned examiner.

While your children are taking the test, a PARENT /GUARDIAN GENERAL INFORMATION PROGRAM will be conducted at 9:30 a.m. in the O.J.H. Civic Center. You are invited to attend so that you will get to know the school where you intend to enroll your son/daughter.

FIRST, SECOND, THIRD, OR FOURTH YEAR
The following applicants are qualified for admission to the Second, Third or Fourth Year Level:
• All promoted students of St. Joseph School – La Salle. (Repeaters cannot be accepted)
• Transferees with a general average of not less than 80%. Applicants must not have any discipline case from the school of origin and must pass the entrance test. Transferees may be accepted if there are available slots.
• Students previously enrolled in St. Joseph School – La Salle with a satisfactory performance and who for some unavoidable circumstances had transferred to another school (returnee).
• Night School students with satisfactory performance seeking admission to the Day School Program provided he/she did not come from the Day School in the previous years.

• St. Joseph School – La Salle Night School students promoted to the next year level
• Transferees or returnees who do not have any discipline case from the school of origin
• St. Joseph School – La Salle Night School students promoted to the next year level
• Transferees or returnees who do not have any discipline case from the school of origin
